The Support At Home and New Aged Care Act Transition Support 2024-2025 (GO7421) Grant is now available for Home Care Package (HCP) and Commonwealth Home Support Programme (CHSP) providers. This one-off $10,000 grant aims to support IT system upgrades and staff training to meet the new Support at Home and Aged Care Act requirements from 1 July 2025.
Who Can Apply?
To be eligible, you must be:
✔ An existing approved provider of Home Care Packages (HCP) – Must provide your Aged Care Approved Provider ID (NAPS or GMPS ID).
✔ An existing CHSP provider – Must have an executed grant agreement with the Commonwealth (valid until 30 June 2025) and provide your CHSP funding agreement ID.
How Can the Grant Be Used?
The grant funding is strictly for IT system upgrades and training to meet the new reporting and claiming requirements under the new Aged Care Act and Support at Home Program. It can be used for:
✅ Paying a vendor to implement required IT system changes.
✅ Wages for internal IT staff to make necessary updates.
✅ Training staff on new claiming and reporting standards.
