It comprises various templates, guidelines, and checklists aimed at enhancing the efficiency and compliance of home care services. The toolkit includes the following components:


Business Case Template & How to Guide: This helps organizations outline the need for a new CMS, the options available, and provide a recommendation for decision-makers. It also includes sections like project timelines, governance, and a detailed breakdown of costs.


Customer Journey Map Template: This template assists in mapping out the client's experience with home care services, identifying customer goals, technology enablers, pain points, and opportunities throughout different stages like onboarding, receiving services, and reviews. This journey mapping aids in improving client and workforce experiences by highlighting where technology can address inefficiencies.


ITC Toolkit - Practical Guide: This practical guide is aimed at helping home care providers understand each step in selecting and implementing a CMS. It includes tips, shortcuts, and details on how to navigate the selection and implementation processes, covering aspects such as systems requirement documentation, RFP processes, and post-implementation governance.


Project Governance Kit: The governance kit provides a template for structuring project roles and responsibilities, setting up governance frameworks, and ensuring proper reporting during the implementation of the CMS. It includes structures for smaller and larger organizations, with defined project roles for the board, steering committees, implementation partners, and functional teams.


Request for Proposal (RFP) Templates: There are two main RFP documents:

  • RFP Template: This sets out the requirements for the CMS, including the project's purpose, scope, pricing, and references. It also provides the indicative timelines and confidentiality agreements needed during the RFP process.
